to join our domiciliary and extra care services across Ellesmere Port and Chester.As a

Care Coordinator , you’ll play a vital role in ensuring the smooth delivery of high-quality, person-centred care. You’ll work closely with care staff, the people we support, families and healthcare professionals to ensure services are safe, effective and tailored to individual needs.This is primarily an office-based position with occasional field support when required. You’ll work on a structured shift rota covering 7am3pm and 3pm11pm shifts, including alternate weekends and on-call duties.If you’re ready to build a rewarding career where your organisation, leadership and care expertise make a genuine difference every day, apply now and join the Making Space team.Key Responsibilities:Plan, organise and manage staff rotas to ensure support visits are delivered efficientlyReview and update schedules to meet changing service requirementsComplete assessments and care plans to maintain excellent standards of careSupport care teams through supervision, appraisals and ongoing guidanceProvide operational support in the field when requiredBuild positive relationships with the people we support, their families and professionalsEnsure services are delivered safely, effectively and in line with regulatory requirementsWork closely with colleagues to maintain high standards across the service

The PersonPrevious experience in care coordination or schedulingStrong organisational and communication skillsThe ability to manage multiple priorities effectivelyExperience maintaining accurate records and documentationA commitment to delivering outstanding person-centred careA full UK driving licence and access to your own vehicle

A Level 3 qualification in Health and Social Care would be advantageous.
